Markers are grouped by difficulty: static background, moving background, and markers crossed by a subject. The scope and schedule come from that split.

The markers nobody quotes for
Ninety per cent of markers in a shot are quick: static surface, locked camera, tracked patch, done. The schedule is decided by the other ten per cent. A marker an actor walks in front of has no background to patch from on those frames. A marker on a wrinkling costume moves non-rigidly. A marker at the edge of frame disappears and returns with different lighting. Those are hand-painted, they take an order of magnitude longer each, and pretending otherwise is how prep packages run over. They get counted and scoped up front here rather than discovered halfway through.

Each marker region is tracked, a clean plate of the background behind it is built, and a tracked patch is applied across the full frame range so it stays locked as the camera moves. Markers that a subject crosses have no background to patch from on those frames, so those are painted by hand. Matched grain is applied over every repair so the patch does not breathe against the surrounding image.

What makes some markers much harder than others?
Three things: a subject crossing in front of the marker so there is no source to patch from, a marker on a non-rigid surface such as clothing that will not hold a planar track, and markers that leave and re-enter frame under changing light. Those are hand-painted and take considerably longer per marker, which is why they are counted and scoped before the work is quoted.

Yes. Marker removal is normally part of a wider prep package alongside clean plates, stabilisation, lens undistortion, camera solves and roto hand-off. Taking the whole prep pass together is usually cheaper and avoids the handover gaps that appear when it is split between vendors.
